To request copies of your medical records, please complete the medical records request form or contact our office for assistance. Our staff will guide you through the process and help ensure your request is processed as quickly as possible.

Requesting Records for Another Physician

If another physician or healthcare provider requires your medical records, you may authorize our office to send the information directly to their office.

Please complete the appropriate authorization form and include the receiving physician’s contact information so our team can send the records securely.

Accessing Your Medical Information

Some medical information may also be available through the patient portal. The portal allows patients to securely access certain health records and communicate with our office.
