Make a Referral
There are two ways to submit an appointment request for your patient. You can either submit a referral by fax or complete the online referral form below.
Referrals by Fax

- Fax (954) 983-4190 the completed form along with relevant medical records, imaging, and insurance information
Referrals Online
- Complete the Online Referral Form
- Upload or send supporting documents including test results, imaging, and patient information
Once your referral is received, our team will coordinate scheduling and notify your office accordingly.

Urgent Referrals
