Make a Referral

There are two ways to submit an appointment request for your patient. You can either submit a referral by fax or complete the online referral form below.

Referrals by Fax

• Download and complete the Patient Referral Form
• Fax (954) 983-4190 the completed form along with relevant medical records, imaging, and insurance information

Referrals Online

• Complete the Online Referral Form
• Upload or send supporting documents including test results, imaging, and patient information

Once your referral is received, our team will coordinate scheduling and notify your office accordingly.

Schedule an Appointment Online

For added convenience, patients may also schedule their own appointment directly through our online scheduling system.

This allows faster access to care while keeping your office informed throughout the process.

Urgent Referrals

For patients with urgent sinus or breathing concerns, please contact our office directly to coordinate expedited evaluation.
